Sliding Fee Program

The Sliding Fee Program allows us to reduce or “slide” the fees for the care you or your family receive at any of the Northwest Michigan Health Services locations. You can apply for the program if you do not have insurance or if you do have insurance and need assistance to help pay for your care.

If you decide to apply, your eligibility is based on two things:

  1. How much income you or your family makes
  2. How many people are in your household

If you qualify for the Program, the amount you owe for your visit will be reduced according to the discount schedule.

How to Apply

It is best to apply for the Sliding Fee Program at the time of your first visit to the health center. The front desk can give you the form to fill out.

You will need to provide “proof” of your income through one of these ways;

  1. Income Tax Return
  2. W-2
  3. 3 recent paystubs

If you qualify for the Sliding Fee Program, you will pay the nominal fee listed below for most medical, dental or behavioral health services.  An exception to this is cosmetic dental procedures or dental procedures requiring outside lab work.  There may also be additional fees for outside lab services.  If you have questions about fees or covered services, please ask.

A message from our CEO:

We at NMHSI want to assure our patients and public that we are here for you. We are responding to rapidly-evolving recommendations regarding COVID-19 to assure the safety of our healthcare staff and community. We remain open and available to serve medical and behavioral health patients. We are pre-screening patients who present with fevers and cough, and triaging them out of our clinics for the most appropriate testing and follow-up. Responding to the recommendations of the Michigan Dental Association, we are now able to provide preventive dental services again, and are scheduling our existing patients whose appointments were cancelled previously. As always our medical, dental and behavioral health staff are available to our patients in our clinics and by phone at (231) 947-0351.

NMHSI is doing drive-through COVID testing at the Traverse City Clinic, 10767 E. Traverse Hwy. and at the Shelby Clinic at 119 S. State Street in partnership with Benzie-Leelanau Health Department and District Health Department #10.  Anyone interested in being tested can call our COVID Test Hotline at 231-642-5292 for an appointment.

To support our patients and community who are suffering from increased anxiety and depression during COVID, we have a COVID Stress Warm Line.  Call 231-655-3867 to speak to a counselor 8-4 M-F.
